Key Responsibilities
- Coordinate care packages for service users
- Manage staff rotas and schedules
- Conduct care assessments and reviews
- Liaise with families and healthcare professionals
- Ensure compliance with CQC standards
Requirements
- Previous experience in care coordination
- Excellent communication and organisational skills
- Knowledge of CQC regulations
- Full UK driving licence
